REGISTRATION

Seating

Club MBA reserved seating is at pre-assigned tables. Reserved seating for the Chairman’s and Sports Luncheon is available only when tickets are purchased in multiples of ten. Preferred seating is given to those who reserve an entire table.

* Note:  The member company name is listed on the badge and the registration list. Registrant badges can only be picked up by the registered individual unless prior arrangements have been made.

Substitutions

Send requests in writing to the attention of "Registration Substitutions" and include a $50 processing fee. See “Five Easy Ways to Register” for contact information.

In order to guarantee that your name will appear on the Advance Registration List, your substitution request must be received on or before August 25, 2006. For your name to appear on the Final Registration List, substitutions must be received by September 22, 2006. After September 22, 2006, all substitutions must be done onsite.

After your substitution request has been processed, you will receive a confirmation letter from MBA with instructions on how to effect a hotel reservation substitution. You must follow the instructions in order to retain the former registrant’s hotel reservation.

Cancellations and Refunds

Send requests in writing to "MBA Cancellations." See below for the refund schedule.

* Received on or before September 22, 2006: Full refund minus $50 processing fee
* No refunds or changes can be made for Ticketed Events after September 22, 2006
* Received between September 23 – October 13, 2006: Refund of 50 percent of the registration fee
* October 13, 2006: No refunds after this date

Program Cancellation

In the unlikely event of program cancellation, MBA will refund 100 percent of registration fees paid. MBA assumes no liability for any penalty fees on airline tickets, deposits for hotel accommodations, or any other fees, charges, penalties or other incidental costs that a registrant might incur as a consequence of a program cancellation.

Guest Policy

All registrants are limited to one guest. A guest is defined as a spouse, significant other, friend or an adult child (18 or over) who is not in an industry-related occupation. Co-workers or associates in the industry may not use the guest registration category.

Attire: Business

Note: On Sunday, October 22, 2006, the Chicago Marathon will be conducted starting at 8:00 a.m. Please try and avoid the delay. Cars and taxis should utilize lower Wacker Drive off of Interstate 90/94 to reach the Hyatt and surrounding hotels.

NO AUDIO OR VIDEO TAPING OF THE PROGRAM PERMITTED

*Use Express Mail for overnight deliveries to meet registration cut-off dates when applicable. Express mail only ensures postmark date for registration fees.
